Employee Form 1095Cs, the Employer-Provided Health Insurance Offer and Coverage form, are now available in Kymera’s HR office.

Current employees can pick up a copy at any time during regular business hours or call internal extension 244 to request one.

The Employee Form 1095C can be sent electronically, via e-mail, only if the employee consents to receive it electronically. Employees wanting to have their 1095C e-mailed to them may request it by e-mailing the hr@kymeramedical.com e-mail using the following statement:
“I am requesting to and consent to receive my Form 1095C electronically via e-mail”.
